I give my child(ren) permission to travel under the supervision of one or more persons authorized by Pali Adventures. I understand that my child may travel in a camp vehicle and engage in various activities. I hereby release Pali Adventures, its officers, agents, employees and the Board of Directors, from any and all liability.

I, the parent/guardian of said minor, do hereby certify that my child is physically and mentally able to participate in camp activities. In case of accident or illness, I hereby authorize the director of Pali Adventures, or an authorized representative thereof, as agent fo the undersigned, to consent to any medical or dental aid (including, but not limited to X-ray examination, anaesthetic injections, medical or surgical diagnosis, treatment and/or hospital care) which are deemed necessary by the treating physician, staff or hospital personnel. I give permission for the director, or an authorized representative, to administer prescribed medications. This completed form may be electronically reproduced for excursions away from the base camp or camp base site.

I will allow my child(ren)'s image or likeness to be used without compensation in any or all photographs, videos, or any other magnetic or other electronic media, for Pali Adventures’ promotional purposes.
